Hip bracing? Surely cost ineffective and a new one for me. Anyone doing this and why? I confess massive bias towards this being completely unnecessary….

The interpretation should not be ‘don’t do THR’ but that caution is wise, more detailed research is required, counsel your patients appropriately and refer on if you are not sure about other factors. Hoping to back this up with our own data soon….

I am, of course not saying this is not a worthwhile study, and that the authors don’t know all this already! We should, however, be careful of blanket denial of potential treatments. All these patients still improved by 3xMIC, and maybe OHS 41 is enough for some. .

There are many non-arthritic causes of hip pain which may not have a hip preservation option and not all will develop advanced OA quickly. Certainly not all will benefit from THR at some point but they shouldn’t all be filed as ‘early OA’. Can’t tell if this was assessed here.

I’s true that if you restrict THA to advanced OA you’re in a safe zone, but there needs to be nuance to this. Undoubtedly there needs to be careful assessment but even from this infographic by that metric you’ll not help the 25% you could have. Note the 25% complication rate too.

I generally don’t do hip replacements unless “bone on bone”.It’s good to see this evidence for THR as we know it’s the case for TKR.

An excellent point. the level of packaging is getting insane. But I know all the hip surgeons are actually thinking ‘what the hell sort of ceramic is that supposed to be?’.

Yo orthopods . I know it's a busy time. But does some of your cool metal stuff come wrapped in lots of hard plastic blisterpack packaging (like our staplers)?. What if I told that we could recycle the equivalent of all Coke bottles sold in the UK per year

Routine MRI won’t add to the diagnosis (which is clinical) or surgical plan (unlike CT), but ‘wasted resource’ is a bit strong! I’ve picked up osteiod osteomas, metastatic seminoma, stress fractures, AVN and PVNS from ‘typical presentations’. That’s its role in my hands.
