What I’ve learned in the 6 years we’ve been researching the #gestationaldiabetes (GDM) diagnostic criteria in Australia - a 🧡.Changing the criteria in 2015 to be aligned with the #IADPSG recommendations resulted in 37% more diagnoses (relative increase).

#AcademicX #AcademicTwitter has anyone else been experiences unacceptable delays with @BioMedCentral journals? We’ve had a revised Systematic Review and Meta-Analysis sitting with Pregnancy and Childbirth since Jan with second reviews apparently received back in Feb!!! 7 months.

Not just a disgruntled customer complaining about flight cancelations or lost luggage (which is bad enough). A real example of blatant profiteering and zero s**ts given from @VirginAustralia with no ability to seek help elsewhere. This is a last resort. Customers are desperate!.

Our work continues…@DrSusan_deJ .and I will continue to advocate for appropriate diagnostic criteria that balances the very real need to identify and manage women with #GDM with not over-diagnosing and unnecessarily intervening with women who do not benefit from a GDM diagnosis.

So what can we conclude? Based on two large studies we have led, certainly in the QLD context, there appears to be little gained from the additional #GDM diagnoses. This, together with other recent studies points towards an argument to relook at how we diagnose #GDM.

The Covid-19 changes to #GDM screening and diagnosis did not appear to produce any unfavourable pregnancy outcomes. The very small increase in caesarean deliveries in 2020 was likely due to factors unrelated to GDM (Caesarean deliveries increased across the board during covid).

Evaluating these changes produced some reassuring results Consistent with additional #HAPO analyses, a fasting blood value <4.7 has a low absolute risk of glycaemia related complications - a possible good screening measure for GDM.

#COVID19 forced a very rapid change to diagnosing #GDM in QLD, Australia (Mar 2020). Essentially, only women with a fasting blood glucose between 4.7 and 5.0 mmol/l needed to have an oral glucose tolerance test (OGTT) to confirm GDM. <4.7 = no GDM; > 5.0 = GDM.

Others have commented that at a population level, we were unlikely to detect changes in outcomes as the actual increase in the number of women being diagnosed is small so the overall impact on outcomes is also small.
