@mattyheron
& I are excited to share Ep2 of ‘Fascinating Fails’ a new series for
@pbsds
&
@pbsterra
!
This episode dives into how we only have general anaesthesia we use today because of some… very serious partying (yes, you read that right!)
Appalled to receive a letter on my daughter (with
#cysticfibrosis
) poor school attendance (due to clinics & illness). Subtle threats incl legal action if we do not take steps to improve her attendance next year. Grossly insensitive & upsetting. You can imagine my reply.
Did you clap for the NHS during covid? If you did, now is the time to support nurses, doctors & other professionals who will soon to balloting for industrial action. Clapping didn’t pay any bills. Clapping doesn’t repay good will. Clapping doesn’t replace staff who are leaving.
I’m an Anaesthetic Registrar & mum of 4. My youngest has
#cysticfibrosis
& is not yet eligible for the jab. She relies on us & those around her. I am 3x vaccinated against
#COVID19
because it protects me & those around me. Caring for others is a basic moral duty
#GetVaccinated
As a hospital doctor I cannot stand by & see what is happening to our GPs. Without them the whole healthcare system will collapse. They are being scapegoated & used to divert blame from year of Gov failings. Once done with GPs we are the next targets

‘Do you know how old you’ll be when you graduate med school?’
Yes, 40
‘Do you know how old you’ll be when you finish your training?’
Duh, yes
50 years old today & still an ST3. Age is just a number. May as well be 50 & doing something I love
Birthday in

#lockdown
Every day I get asked what area of anaesthesia I want to specialise in. Answer is. I don’t know yet.
Got me reflecting on how long it took to get to this point. Someone said you must share your story. Feels like over sharing, but bear with me. Long thread!
24 years ago I gave birth to my oldest child. Had an epidural. Vowed there & then to become an anaesthetist. Wasn’t even a doctor. So it took quite a while!
Today I collected my
#FRCA
from my F1 & F2 supervisor &
@RCoAPresident
Dr Fiona Donald. How marvellous!
#RCoADips
Nurse on the ward. ‘Are you a surgeon?’
‘No, anaesthetist.’
‘Oh, I was looking for someone to prescribe. You can not.’
Anaesthetist are doctors, just in case you ever wondered!
I’m working nights next week. My petrol tank is empty. Bus or bike not feasible. Pumps empty. Thoughts please
@BorisJohnson
on how key workers will get to work?
#panicbuying
My Uber driver once was a surgeon from China. He had 18 years surgical experience. Was unable to get through necessary GMC admin &££. Drove Uber while his wife did GP training. He resigned himself to this fact, relished his supportive role. But honestly, what a loss
She loves school and goes in despite feeling not up to it at times. If a child is not at school don’t send blanket letters. Find out more. Be sensitive, esp to children with progressive medical conditions
#SchoolAttendance
As a parent of a child with a significant medical condition the impact of this letter cannot be underestimated. Caring for her takes hours each day, hours of pharmacy visits, medication ordering, hospital visits. You do the best that you can. You try to live life normally.
It’s a v busy shift. The surgical reg was downing an ice cold Fanta (Lilt). ‘Oh how delicious’ I said, ‘I’ve not had one in years. I’m going to get one later!’
A few mins later the Reg appears with an ice cold can for me! Could have cried. Made my shift. Thank you 🙏🏽 🤩⭐️
I passed my Final FRCA!! No more exams ever.
Since 2004 my kids have had their mum doing some sort of exam. They are soooo happy too.
Massive thank you to everyone who helped with practice, support & so much more. Giant thanks to
@DrPaul_Watson
- our household is now exam free!
Your most embarrassing moment in medicine?
I have several that I’m not sure I could share 😳
Whispering 69 to the dean of the medical school in front of the entire year is one. I meant to say 99. He replied ‘only on Saturdays’
TBH I’ve blanked the details of this
Asked my white doctor husband how many times he’s been mistaken for the domestic. Never.
Me? Too many times to count. I have no issue with these important & vital jobs, they are valued team members. The issue is assuming a brown woman doesn’t have the capability to be a Dr.
I was recently given some ‘constructive’ feedback on my personal appearance. Comments that were in-fact deeply hurtful. But just to be sure, this is my messy hair in its natural untamed state ICYMI. We need to accept that hair can be unruly & frizzy. It is mine. I own it. Thanks
I am a ‘junior doctor’. We are not children or immature. We will put you on life support, send you off to sleep for an operation, perform surgery, treat your heart attack, diagnose your illness, deliver babies, prescribe life changing medications & save lives. Waiting lists, NHS
I made a significant error of judgement today in ICU. I apologise to my team & everyone affected. I fully understand if this goes on my MSF. To take a microwaved mackerel meal into the radiology meeting & confined space was totally unacceptable. It was selfish & misguided...
I’ve actually been holding in my dismay & frustration for a while now. I think many NHS workers have. But no more.
People - Stop sharing the utter nonsense & dangerous peddling of lies.
@twitter

When you say you don’t know who to believe on Covid, you’re treating the testimony of frontline NHS doctors and nurses like me as no more believable than anonymous Twitter trolls.
Can you imagine how that makes us feel?
We’re your NHS.
Please trust us 💙
@EmergencyBod
Oh my gosh so similar! I arrived in primary school in Glastonbury with my frizzed up hair & no friends. A little Ghanaian girl out of place. This girl came up to me & said do you want to play? She is my best friend Bev & we’ve been friends for 48 years!
The perpetual infantilisation of the medical workforce must stop. When we people get it, people are
#juniordoctors
well into their 30-40s, & 50s (like me). This article by
@gabyhinsliff
shows why the term must be ditched. Being a doctor despite age requires maturity beyond years
Doctors - how long & what qualifications have you earned that enables you to do your job?
Me -
3 yr BSc
1 yrs MSc
6 yrs med school MBChB
2 yrs (FTE) foundation doctor training
8 yrs training in anaesthesia
FRCA diploma, numerous professional courses
And lifelong learning/CPD
#JuniorDoctors
working an 8pm-8am nightshift, have worked 8 hours of one day right? So why is this post nights component of that day called a 'day off'?
How did this come to pass?
@BMA_JuniorDocs
Confirmation of death is something all doctors do. It’s happening a lot right now.
I often wonder what other healthcare professionals do at that moment. Not the actual ‘no carotid pulse palpable etc’ but the unspoken stuff. I address them by name (silently) & bid them goodbye.
Diversity of background, age & experience mean we all bring something different to medicine. Rather than discouraging & judging I hope we are more supportive & open of our differences.
#MyPathToMedicine
The Primary FRCA was the hardest exam I ever took. I took 4 attempts to pass written, 3 for OSCE & 2 for viva. It took years of my life. These example questions from RCoA demonstrate it’s rigour. The Final was another challenge to overcome. The exam is humbling.
#TeamAnaesthesia
So my daughter ‘graduated’ today! Congratulations on your achievement
#NCLgrad
But…..
11 years ago at my graduation you were a little terror. I smiled & thought I will do that at your graduation.
Today, my savage (back) turn.
#RevengeIsADishBestServedCold
I remember intubating people one after another. The PPE, the plummeting sats, the desperation. Much of it has been buried in my mind. But can be recalled in an instance. I shall watch. Thank you Rachel for telling the story.
Weighing up my post nights decision. I may or may not have gone to Costco and come out with a giant 3-scoop gelato and a flat screen TV
#Costco
#postnightshift
2004 started medical school. 2014 started ACCS Anaesthetics.
My kids have only ever known me in training. They were shocked that I am still in training after 20 years & still called ‘Junior’. 2024 I CCT.
It’s an epic journey. Twenty years in training. 20!
Finally. It’s here! Years of campaigning. A true miracle drug. Got to admit I cried as I held the box in my hand. What a wait we have had.
However, so many people in the CF family didn’t make it to get this medication, that makes it bittersweet.

Being an
#anaesthetic
registrar is mostly amazing & rewarding. I do wonder if folk have a good idea of what anaesthetists do. With a looming strike & implications that all NHS staff need to work ‘harder’ let’s just run through some things we do. Do please add your experiences too
‘I boshed in the tube. Grade 3, MAC 3. Mate, 26cm at the teeth. Art line in. BP was like 45/20 so pushed some adrenaline. Central line in within 2 mins. Life saved. I’m pretty good to be fair.’

I’m woke. Who else is woke too?
I also like Tofu, avocado, 20 mph speed limits, trans rights, human rights & all the rest of it.
Woke isn’t an insult for me. I find it funny that people use it as such.
Finally, nine years after graduating med school, three years of F1, two babies, extra time in anaesthetics (extension, exam failures, fellow) today I can finally say ‘I’m a Registrar!’ I’m not even sorry I keep repeating this. I never thought this day would come!
#TeamAnaesthesia
In 2010 I was told I didn’t stand a chance at an anaesthetic career.
In 2023 I gave the address

1.5% of UK anaesthetists are black. This does not reflect our patient mix. Diversity is vital…
Well the pandemic certainly has some odd things. Consultant interview in the bottom of the garden.
No matter, as
@DrPaul_Watson
nailed it!! Congratulations 🥳 - so proud!!
Consultant in Anaesthesia & Persistent Pain management
will be when you finish your training? Yep, I do 😂.
It’s taken 23 years to get to today & I am old enough for Saga holidays.
Not sure if this is of interest at all. But obstacles & discouragement make the journey long & winding. One step at a time... keep on going.
You don’t compare the pineapple or banana to the ubiquitous apple. Apples themselves come in many varieties. You look at a varied fruit bowl with excitement & appreciation of the differences of each fruit. We should look at people with that same appreciation

Im sure I’ve done a shift with Shruti. I know her. I’ve been her.
Well I haven’t. But her acting is so brilliant I feel like I have. Ambika Mod deserves as much accolade as Ben Whishaw
#ThisIsGoingToHurt
Shruti from This is Going to Hurt was so fantastically played. The TV show feels a lot darker than the book, but special in its own right. Incredible. (And a bit hauntingly relatable as a clinical student).
The myth that
#anaesthetists
pick the specialty ‘because you don’t need to talk to your patients’ persists. Gaining trust in a patient to render them unconscious & take over their vital physiology requires effective communication. I’m about 3-4 years into my training. But...
Having seen the tragedy & trauma in
#ThisIsGoingToHurt
when someone says ‘I’ve just come from a bad arrest’ ‘something bad happened’ etc, instead of ignoring it stop & ask more. Listen. This may be a hint that they need to talk about something. Look after each other.

I am aghast. Sadly these punitive exclusionary practices seem to persist. They are decidedly ableist & discriminatory to those with chronic illness & disabilities. Must be in violation of Equality rights

When I was a foundation doc in a nightmare job, child in nursery (with 6pm pick up). Official hours were 9-5. A ward round started at 5pm, expected to stay. I couldn’t stay late so I would come in 1hr early to ‘pull my weight’ This was not noticed only that I left early at 5.45pm
She was in her 40s.
A junior doctor.
Older than all her peers, she was different.
She was the one who left on time.
Eyes rolled.
People tutted.
She wasn't committed.
She lacked dedication.
Before you hit send on that passive aggressive toned email to trainees stating that someone must step up to cover the rota, disappointment that no one has submitted a poster/audit etc, saying the Cons will have to step down etc, remember trainees have already worked above..
Anaesthetists work with highly skilled Operating Department Practioners (ODP/ODA/Anaesthetic Nurses) like Russ here. We cannot do our jobs without them. Looking forward to sharing their role next week on my

Anaesthesia is such a supportive specialty as a whole. I was lucky as an F1-2 to be allocated Fiona Donald as my ES. She continues to be my mentor & has supported & encouraged me the whole way. Plus, brilliant CTs such as
@edcur1
, Hugo Hunton who taught & supported me. Thank you
VP Fiona Donald spoke of the unique challenges faced by anaesthetists in training during COVID.
“Trainees are not junior student doctors unburdened by responsibilities. They balance home & family life around this often exhausting, demanding & antisocial job.”

My kids simply don’t get the significance of seeing Jason Donovan getting a doner from Jason Donervan.
Since early Uni days I never thought this would ever happen 🤣
I, however, was at work
#SaturdayNightTakeaway
Listening to all the chat among striking doctors I think that the pure bubbling level of discontent is drastically underestimated. People are literally fed up & actively looking at other careers or countries. It’s very serious. Who will look after you or family in the future?
The work of
#JuniorDoctors
, like many say can’t be compared to other professions. True.
The doc who put a breathing tube in after a young person was attacked , a junior doc
The doc who put an epidural in a labouring woman so she can birth her child born sleeping, a junior doc
The infantilisation of the medical doctor workforce has to stop. The term
#JuniorDoctor
should be ditched ASAP.
It’s time that doctors were seen as competent adults making life & death decisions contributing to the healthcare of the nation.
Is it a child looking after you?
The training path is so long in many specialties that by the time you add in family, exams & life events the age of a junior doctor is in middle age. Please consider this

In my 13+ years as a doctor I have worked many many night shifts. A huge amount of my learning as an independent doctor comes from these shifts. They were vital. What do others think?
Interesting chat last week with VERY experienced senior female consultant anaesthetist. Chatting about how she too gets talked over & down to by male SHOs/Regs. I was shocked that happens to her too. Men, if you are someone who does this, it is noticed!
I qualified med school in 2010. Still in training after 12+ years. I’ve four children, two who have graduated university themselves, I’ve worked in another career prior to medicine. I owned my own business. I am 52. Still people patronise & infantilise me.

This portrayal of GPs as somehow to blame for the crisis is orchestrated & harmful. The NHS & GPs have delivered care during the pandemic despite immense challenges. Once we were all heroes. Now heroes are falling. Enemies now.
