yes i am well aware of the fact that this would impact gameplay and make it torturous but if youre gonna give your characters major physical injuries for the purpose of shock value, dont pussy out on forcing them to face the music. either that or dont do it at all.

these injuries are all integral to the plot/the character and they have literally no detail whatsoever. it also goes to say that r* generally doesnt handle emotional trauma that well either, but thats a whole other conversation for another time.
