The Great Debate: Why MCTD is indeed a unique disease: IIM/SLE/SSc features: high titer ANA/speckled; high titer U1RNP, rest of ENA neg; CNS/renal involvement are not typically part of the condt. sometimes a subgroup of pts may evolve into separate entity over time #ACR24

SCLEROMYXEDEMA can look quite similar to SSc! (may also see dysphagia & arthritis). Look for papules & involvement of Ears & Back. Specify to pathologist s/p bx that you’re looking for a mucinous deposition dz. Dermato-Neuro syndrome a/w encephalopathy, seizures #ACR24

Melanin may mask erythema; De-pigmentation is seen more often in DM/SSc skin involvement in those of color. Low threshold to request for skin bx to help prevent delays in dx. In some SSc pt’s with salt/pepper rash, pigmentation may return as a response to tx - Dr Okoye #ACR24.
