thank you to @Autism and @NHSProviders for offering insight into these stories. And thanks very much to Shelagh Sheldrick, who generously spoke to me after her child was tragically failed by several services in Sussex.

I think this gives an indication of the scale of this largely hidden crisis, where some of the most vulnerable fall through the gaps of overstretched and poorly-connected services. It is the job of ICBs to facilitate joined-up care. I hope they read these reports and learn.

In total, 24 of 53 PFD reports sent to ICBs covered primarily focused on mental health services. This is far more than the next most common theme (emergency care) which received 11 reports.

In one case, a 29-year-old patient died after enduring nearly a month in A&E, followed by another month with inadequate community care. This harrowing report kick-started my analysis. https://t.co/iFN0mQwP3v

A coroner has ordered an integrated care board to fill gaps in its services and address mental health bed shortages following the death of a 29-year-old with autism and ADHD who spent 26 days in A&E.

In some cases, patients waited weeks for urgent crisis appointments. In others, they were rejected by multiple organisations because they had a learning disability alongside their mental health problems. When services are oversubscribed, they can 'pick and choose' who they admit.

We've heard a lot about very long waits for autism and ADHD assessments, but this is just the start of the journey for many patients. Those in these reports have faced intolerably long waits for care from multiple agencies - mental health, learning disability and other orgs.

I analysed every PFD sent to an ICB since 2022 (when they were launched). By far the most common themes were mental health failings, often compounded by failures in services for additional needs like learning disabilities.

One year ago today, I was 9th in the ambulance queue outside a West Midlands hospital endlessly waiting to be ‘given permission’ to bring my patient inside two hours after my shift finish time …. this evening I was *25th*!!!! It is intolerable.

it's been a pleasure! Dimi gave me the chance to work on important stories that might have struggled to find a home - pieces tied together by a mode of reporting (data, scraping), rather than a specific beat (health, politics). Excited to see what's next for you & @theleaduk!

Experts say its an "interesting" addition to the literature on the disease, but any talk of navigating etc boosting your hippocampus is speculation and not evidenced in this study, which did not look at any brain scans etc.\ so don't turn off your satnav just yet...

Driving taxis is being widely reported as protective against #Alzheimer's disease, on the basis of a a @bmj_latest study into death certificates. FYI the study does not actually provide evidence for this particular claim:

Could the navigation involved in these jobs cause changes in parts of the brain related to the disease? Experts aren't convinced.

great report on the science around seed oils - tldr; don't worry about them. alternatives like olive oil etc can be expensive, and some (e.g. coconut oil) are v high in saturated fat. no need to feel guilty and switch over to pricier oils at home. https://t.co/E5kzLW6VKT

Seed oils are blamed for a number of diseases. Consumer Reports looked into the science behind those claims.
