This is a disaster for @RCPhysicians . Errors can happen but how they respond now will be the litmus test. Why so long to identify the error?.What measures to prevent recurrence?.How to support and remedy training for all those affected?.Investigate and publish if patient harm.

Anaesthetic care (these often with higher risk profile) results in few complications and cheaper. The only reason for not doing it is lack of availability.
Should non-anaesthetists provide procedural sedation for interventional radiology procedures and endoscopies?. Anaesthetist-directed sedation was assoc w ⬇️ risk of adverse discharge to a nursing home and a shorter postprocedural duration of hospital stay.
