I just learned that tonicity and osmolality are not the same thing, and that serum osmoles can be elevated from azotemia but it’s hypotonic. Why do we check serum osmoles then? Would there be a way to only measure effective osmoles to better measure tonicity? #askrenal

Today's consults: 1) anabolic steroid case with AKI and ALF, 2) Nephrotic synd with Hep C, DM, and family hx of ESRD, 3) HFrEF resistant to diuretics, trial for hypertonic saline induced diuresis, 4) AKI on CKD, recently started on hydralazine Tons of discussions on GN and HF!

Had a patient on nights with mild CKD and persistent NAGMA I could not explain. But I finally made the connection that he has a ureteroileal conduit which from what @Proximal_Baxi taught me in clinic alkalinizes urine which can cause NAGMA 🔥 #renal

Highly recommend using employee directories with pictures. I constantly have ours pulled up so I have a face to the name of whoever I'm interacting with over chat, phone, etc. It would help us all be much kinder with one another.
