We all want “gold standard evidence” and can reasonably move toward gathering better data. But if your goal is to exclude by RCT any chance of every harm over the entire lifespan, you’ll never accept any medical intervention. Which is just fine for our current leaders.

“Peer nations do it” this way ignores this: Countries that use selective hep B birth-dose policies usually have >95% prenatal screening, centralized records, and near-universal access to care. The U.S. has none of that. Importing their policy without their infrastructure
