This was followed by wide open MR and profound hypotension. The key is to expect this and let your anesthesia know, as well as being quick with septostomy and valve deployment (sometimes having IABP can help)

78 year old with prior Edwards 32 mm physio ring repair, comes in with stenosis and MG of 14 mmHg. Neo-LVOT of 160 and high risk for LVOT obstruction with AML length of 2.8 cm

Sometimes u don’t have a prior stent in LCx Ostia. So prior to stenting across LCx leave a wire there to mark it. Or deliver a short stent to ostia via🏓. Facilitates visualizing LCX take off prior to fenestration @DocSavageTJU @MitulPPatel1 @adityadoc1 @evandrofilhobr @BaoGTran.

Original case: 56 male, CCS III angina, AWE, Ca+ > 5 mm depth on IVUS with scattered nodules —> aggressive modification followed by PCI. (Old cavity fistula noted)

#ICFIT tip: use a longer balloon (27-30 mm) to advance Guidliner over it. Tracks much better than shorter balloons (12-15 mm).

Interesting case done recently. 85 y/o 👵🏼, hx of Afib, SSS s/p PPM presents with CHF, SOB on effort. LVOT gradient 17 mmHg goes to 28 mmHg with valsava. Resting HR 65 bpm. ERO 0.53, Mitral valve MG 3 mmHg, 3D MVA 4.9 cm2.
