Referral for outpt PCI- l met first time in pre-proc. 63 yo non DM totally asx, daily exercise at gym. Coronary Ca++ score obtained for fam hx=2217. ESE 12 METs, no sx, max HR 70% predicted on b blocker. Referred for this cath due to “non dx ESE”. Now what?

Culprit was left main. Key ECG finding was ST⬆️ in AVR which is bad news & esp when AVR⬆️>V1 suggests LM dz (Yamaji JACC 2001; 38: 1348-54)

When using Guideliner, an important trick to avoid causing a dissection is not to advance the GL alone over the guidewire. Instead put a balloon thru it to center the GL & prevent GL tip edge from➡️dissection
