Agree that results are suboptimal with muscular VSD devices. We have had some success with BASSINET (hybrid surgical or percutaneous). Requires direct RV access.

Many still ask if it will reverse the shunt and cause hypoxia. It will not. I find most post infarct VSD shunts ratios close to 3:1 (2.9 in this case). The 5.0 Impella will reduce it to less than 2:1 (1.7 in this case).
