@ehlJAMA A related problem is the dying of the physician-scientist. Study sections are now populated >95% PhDs. Vanishingly rare to find MDs on them. Which means the science being funded is increasingly dissociated from actual health impact. This is a problem decades in the making.

In my 26 years as a physician, I have trained under, collaborated with, and worked beside some of the most brilliant minds. And the smartest of them always knew that they didn't have all the answers, and not only could, but wanted to, admit when they didn't know something.
