A Ph2 study assigned TC survivors to 12 weeks of HIIT vs usual care. Patients on the HIIT arm decreased modifiable CVD risk by 20% compared to SoC. 6/7

So what can we do? Several studies (including one interventional trial) have found that consistent, high-intensity exercise can blunt the cardiovascular effects of chemotherapy for TC survivors 5/7.

Meuleman et al developed a vascular fingerprint to identify patients at highest risk of CVD post TC. A high vascular fingerprint (combination of BMI, smoking, HTN, HLD, and DM prior to chemotherapy) identified all their patients with arterial event 4/7

This is likely the result of endothelium dysfunction from chemotherapy, which can be influenced by other risk factors. Lubberts et al found that obesity/smoking (HR 4.6) and ⬆️ lipids (HR 2.9) increase CVD in TC survivors 3/7.

Going back to 1987 we recognized that chemotherapy drugs for testicular cancer (TC) could lead to vascular disease. An experience from the University of Minnesota identified 5 young patients with premature vascular mortality following platinum based chemotherapy 2/7
