✔️Consider "Blow out myotomy (BOM)" in patients with recurrent symptoms after myotomy. BOM is defined by an increase in luminal diameter of >50% in the distal esophagus on esophagram and is more commonly associated with heller myotomy and type III achalasia. 5/5

✔️Patients in recovery who do not require opioids for analgesia can be considered for same-day discharge. 4/5

✔️Consider intra-operative EndoFLIP for intraprocedural assessment of adequate myotomy. A post-myotomy DI >2.8 is correlated with clinical improvement. 3/5

Learning Points:.✔️Shorter myotomy (3 cm) for type I and II achalasia has shorter procedure time, similar clinical efficacy and potential decrease in post-POEM GERD compared to conventional (6 cm) myotomy. 2/5
