Becoming a heart surgeon was hard. I do so as I reflect on how drastically my environment has changed around me. Growing up as a little boy in Cameroon, it was daring just to dream I'd find myself in a position to simply be alive at this stage, let alone seek to accomplish more.

Key is to have many “tools” in the arsenal. No one technique can be used universally. Also, none should be simply discarded as there are subtle technical points with all of them.

There are many useful ways to approach small annulus. Posterior patch, Konno, Freestyle, even Ross is a good option in some small annulus patients. Even a sutureless valve can serve a handy bailout.

A key technical point is not to oversize the valve and not to travel too far up the patch. Riding sutures high on the commissure will cant the valve and can’t distort the LVOT. However, keeping stitches low will maintain geometry and allow ~ 2 sizes up.Which is fine in most cases.

The above patient has a BSA of 1.9. Root measured 21mm. Posterior rectangular “Yang patch” used to enlarge to a 25 Konect. MG 5 mmHG post. LVOT wide open.
