Complications in the cath lab can happen, and that’s the time you need kit quickly. We have designed a new complication trolley, standardised for each of our labs so we can access all the equipment needed swiftly without having someone leaving the room. @besagemo1 @bakerCharlot

We don't go to work wanting to do harm, but with error it can be easy to point a finger & blame With a JUST CULTURE, incidents are investigated to determine WHY they happened rather than WHO was to blame Importantly what can be learnt & shared to reduce an error happening again
