heavily calcific RCA , Severe mid segment stenosis ,1.5 mm🎈 uncrossable, failed PCI in another hospital .AL1 7F guide, 1.75 burr, Microcath couldn't pass for wire exchange, so wedged proximal to the lesion Rota wire passed ,predilatation with 2 mm & 3.5 mm NC ,then 4x28 DES

Our last TAVI case of a relatively young 62 y 👩 who had severe AS with an area of 0.8 cm2 .She had other medical problems that increased the risk of SAVR .She has SLE and CKD. This also prevented CT aortography. All measurements were set by TTE, LL duplex ,🎈 interrogation
