I have attached all the relevant documents, including a copy of the FIR, my medical prescription (discharge summary) and an application citing the incident and reason for the delay, to support my claim and request for condoning the delay in filing the claim intimation. @CholaMS.

As per the doctor's advice,I was required to undergo complete bed rest, which made it challenging for me to immediately notify Cholamandalam Insurance Co.about the claim. As soon as I was physically able to, I promptly intimated the claim and submitted all the necessary documents.

It is to bring to your attention that there was a delay in the intimation of the claim. I was unfortunately admitted to the hospital and underwent surgery when the incident occurred.
