We highlight the continuing challenges of inferring antimicrobial resistance from sequence data and our study suggests that chloramphenicol could be reintroduced as a reserve agent for critically ill patients with ESBL-E infections in Malawi and similar settings.

However, many phenotypically susceptible isolates had a genotype indicative of resistance and carried chloramphenicol acetyltransferase genes. We used a combination of functional assays, experimental evolution and genomics to investigate these genotype-phenotype mismatches.

Could chloramphenicol play a role again in the treatment of MDR Gram-negative infections?Susceptibility to chloramphenicol in E. coli and Klebsiella spp. has re-emerged in Malawi, where its use had been discontinued since 2004, raising the possibility of its reintroduction.
