Useful reflection ⬇️. We’ve thought about how to use our #patientstories project to keep a patient’s goals and #values central to care. We initially offered it at point of discharge but hearing and sharing their journey earlier in an episode can help with this! #PatientCare.
The authors detail a useful reminder on shared decision making, where a person’s values and wishes are at the core, from the beginning, not a further consideration when they differ from the SLT recommendation.
