Next up: DOTS trial studying dalbavancin vs SOC for SAB (33% MRSA). Primary outcome DOOR at day 70, designed as superiority analysis. Good mix of infection sources including endovascular. @ESCMID #ESCMIDGlobal2024

Currently, MALDI-TOF/ 16 s-RNA sequencing helps identify this organism. As this is skin flora, interpretation of pathogenicity/ role in disease process is carefully interpreted. Our patient is receiving Dalbavancin for vertebral osteo treatment targeting this organism.

"... isolation of C. tuberculostearicum from human clinical samples is underreported since this species is not included in the API Coryne database and is therefore systematically misidentified as other lipophilic corynebacteria such as C. macginleyi or C. jeikeium."

C. tuberculostearicum was isolated from 8 biopsy specimens, 3 aspirates, 3 deep-wound swabs, 2 superficial-wound swabs, and 2 urine samples in this study. All of the strains were sensitive to vancomycin, linezolid, and daptomycin.

#IDtwitter 45 yo w/ L4-5 discitis/OM now, h/o L5-S1 OM (destructive changes at this levelπŸ‘‡πŸΌ) Rx 2015 w/ vanc/cefepime x6wk empirically (Pt not open to biopsy then).Unrelenting LBP continued->multiple steroid injections->Corynebacterium tuberculostearicum on biopsy this occasion!
