@RibianszkyA @MGBpathology @MGHPathology Annamária, these are all great differentials. The tumor is NKX3.1+, NKX2.2+, CD99+, exhibiting similar morphology with the tibial primary of mesenchymal chondrosarcoma. Absence of hyaline cartilage in the metastasis can pose a diagnostic challenge.

@Jwan_Alallaf Great thought. The staghorn vascular pattern, IHCs and prior history are consistent with metastatic mesenchymal chondrosarcoma to the kidney.

@Jaysharav53 Great thought! No hyaline cartilage is present in the renal biopsy makes the diagnosis of mesenchymal chondrosarcoma challenging. The IHCs and prior history are consistent with metastatic mesenchymal chondrosarcoma to the kidney.

@TingZhaoPathdoc @MGBpathology @MGHPathology I forgot , actually the staghorn vasculature is also a sign of mesenchymal chondrosarcoma ,它带着前生的记忆来到了kidney 🥹

@Kawtherkathy Great differential. Primary Ewing sarcoma can rarely arises in the kidney. But this is a different one. Check out the answer I posted.

@TingZhaoPathdoc @MGBpathology @MGHPathology Looks like a starry sky pattern, could it be Burkitt lymphoma?

@n0r0zha @MGBpathology @MGHPathology The small round cell component does raise the differential diagnosis of lymphoma. I think the cells with pink cytoplasm adjacent to tumor cells are tangential cut of endothelial cells, not tingible body macrophages.

@TingZhaoPathdoc @MGBpathology @MGHPathology Without a prior history, could a definitive diagnosis be made?

@GersonPaull_MD @MGBpathology @MGHPathology Dr. Paull, great question! NKX3.1, if positive, may be helpful to differentiate from other round blue cell tumors (PMID: 33302222). If without history, I would definitely want to send the specimen for gene fusion test.
