What does it do?. States will apply with their own plans to be reviewed by CMS against high level goals like improving access, training / retaining a rural health workforce, and technology modernization. Not a giveaway, CMS is going to be monitoring this closely for results.

Of surviving rural hospitals, close to 50% run with negative operating margins and cash on hand is thin. I'm not a fan of blowing out the deficit, but if you wanted rural hospitals to survive (especially in light of Medicaid tightening), something had to be done.

America's rural hospitals have been getting smoked for years. We've lost ~200 in the past 2 decades. But good news: while you were arguing about Medicaid, Congress slipped the Rural Health Transformation Program (§71401) into the OBBB, promising $10B/year to rural providers.

Medicare alone is a bigger budget than all of defense. We literally spend more on insulin and glucose monitors for seniors each year than we do on munitions.

I just heard a stat that, if true, is very revealing about the state of intramural research at NIH: the median age of an intramural investigator is now >60 years old. Many scientists do their most creative work when young. Intramural is ~10% of NIH budget, not insignificant.
