As a Social Prescribing Link Worker we provide 12 weeks of support. If you live in sefton & registered with a Sefton GP surgery you are eligible for this fantastic support! If you would like any more information please contact your GP surgery for a referral! #socialprescribing

Thank you @BrighterL for delivering the #training session to our GPAs across #Southport and #Formby. This is a new role in the surgeries, and they have now been trained up to be #SocialPrescribing Champions. We're looking forward to working together more!
