Histologically a fibrocartilage, the plica presents as a triangular hyperechoic structure in the HRJ space, positioned below the level of capitulum and radial head. Moderate vascularisation and abundant nerve endings, thickness of 1.7-2.2mm in asymptomatic individuals.

👉Radiohumeral synovial plica: A fold in continuity with the capsule, blending with the CET, 77% prevalence in asymptomatic patients. The lateral fold, visible on ultrasound, appears in 5-20% of cases.

Potential entrapment can occur due to mechanical stress, an accessory brachioradialis muscle or space-occupying lesions like ganglion cysts or intramuscular lipomas within the supinator muscle (further investigation for liposarcoma)

👉Deep branch of radial nerve: At the arcade of Frohse, the nerve shows an angulated course during supination, which should not be mistaken for entrapment.

Entrapment can occur due to mechanical stress, acute impact, ganglion cyst, anconeus epitrochlearis muscle (>18% prev. in EU population), Struther’s arcade (52.6% prev.) heterotopic ossification, or posterior HUJ effusion. More variants are found distally at the wrist/hand region

👉Ulnar nerve: Through the retroepicondylar groove, with the MEOL forming the roof. At the FCU heads level, true cubital tunnel, with Osborne’s fascia or humeroulnar aponeurotic arcade (HUA) forming the roof.

👉Median nerve: Three main variants—(1) deep to the medial fascial plane, near the joint capsule; (2) piercing the pronator teres muscle; (3) piercing the brachialis muscle, close to the joint capsule.

👉Accessory ossicles: Six types with a prevalence of 0.77%. Types I and II, located in the coronoid and olecranon fossae, can limit flexion/extension, often requiring surgical intervention. Differentiate from calcifications by smooth cortex and oval/circular acoustic shadowing.

Differentiate from osteochondroma (cartilaginous cap). Lateral supracondylar process is rarer and may compress the radial nerve or radial collateral artery.

👉Medial supracondylar process (prevalence 0.7-2.5%, more common in males, linked to CdLS). Occurs with or without the ligament of Struthers, or the ligament may exist on its own. Potential entrapment site for the median nerve and brachial artery, or rarely the ulnar nerve.
