Having started my career in basic neuroscience research, I have always understood Alzheimer disease to mean the pathophysiology associated with amyloid plaques and tau tangles. As a clinician, I appreciate that the relationship between Alzheimer disease and cognitive impairment

Don’t lie!! Do you just say a lesion is in the white matter & move on? Do you just corona radiata or centrum semiovale & hope no one asks questions? That’s like saying a fracture is in “the leg” & moving on! Just like cortical regions have function, so does the white matter!

This may be obvious to some but I'm now convinced: fierce competition for grants and papers is leading to low societal ROI (return on investment). Why, when competition should mean only the best-formulated and executed work is accepted for funding and high-profile publication?
