PSA screening media coverage is currently very poor. Various charities are not helping but instead adding to the confusion and hype. The Times today carries another high profile individual story of how they were "absolutely" saved by screening with PSA testing.

Wow, I started out as a medical student @UCLMS twenty one years ago. I remember the hospital being built and opening. I have fond memories of it as a medical student - so many excellent teachers. What a remarkable institution it has become.

The main message of the history of medicine is that observation should always trump theory. Theory leads people to see only what they want to see. I'm not talking just about Galen, the 4 humours and so on. I'm talking about the present.

Such patients quickly turn to others with their condition & far outstrip doctors in understanding, often (e.g. in Addison's) saving their lives in the process. Generative AI will transform diagnosis and connectivity for these patients &millions more (incl Long Covid) @crblease

One lesson I have learned as a doctor is that a disease is only rare if you don’t have it. For the person living with a disease, it's their daily reality. Every patient deserves understanding and good quality care from the NHS regardless of how uncommon their condition is.

The plan promises that patients will carry the power and their feedback will be impactful, even used punitively against providers. But is this gaslighting the majority of patients who sound like they'll be shut out altogether in this new world? Who do they feedback to?

So it's now harder to be seen in primary care, & even less likely you'll see a GP expert generalist once you make it past the app. Your GP then feels you need to see a specialist but two thirds of the time an AI tool will screen you out & give you automated info/ digital advice.

This is so true. When a student kept interrupting a colleague taking a history the other day, he said to him "for a GP, the history is like a scalpel to a surgeon" it's our superpower and how we make difficult decisions fast and safely. It is a complicated and skilled process
