Yesterday, I presented my Senior Grand Rounds on the impact of race/ethnicity in HFrEF presentation, management, and candidacy for advanced HF therapies. Here are some key takeaways:

6. This figure from an AHA statement on Implementation Science by @KBreathettMD et al. shows how complex initiatives to improve disparities are. You can’t address one part without considering another, and it will require providers and facilitators to buy into these initiatives.

5. Though disparities are apparent in allocation of therapies, outcomes after AHFT implementation are largely equivocal among different race/ethnicity groups.

4. This study by @KBreathettMD et al. was pivotal in discerning areas where we as providers may be introducing bias into the advanced HF therapy allocation system.

3. Once HF worsens to the point of needing an LVAD or HTx, patients undergo a rigorous evaluation process, however this is still subject to provider bias, particularly as it pertains to psychosocial evaluations.

2. Different race/ethnicity groups present with higher prevalence of some key risk factors, and for Black patients, at a younger age.

1. Race, a social construct, and health are intricately tied because of historical disadvantages to non-White groups. This flow diagram from the AHA statement on structural racism by Churchwell et al. is demonstrative of this fact.
