Had a bad complication recently & it’s been hard to think of anything else… today in clinic 3 patients in a row hugged & thanked me for what I did for them. A good reminder that we do a lot of hard but great things. I have to recall those when times are tough.

Lesson OTD: Surgery is not just about the operation. It’s taking care of the whole patient. Ensuring they have the tools needed to succeed. A technically perfect amputation won’t heal w/out proper nutrition, glucose control, rehab plan, etc.

4+cm common iliac aneurysm with length too short for IBE. Used the Mayo up and over technique after gaining length by placing the EVAR first, then the branch device. Happy with our result.

72 yo w/ rest pain. CT shows occluded iliac w/ prior undersized stents. Prior endart now aneurysmal. Did open repair of CFA & PFA, crossed occlusion, VBX in CIA, SE in EIA. 1 month out now with normal ABI & surveillance duplex… & pain free! 🤩

Transparency in key. Everyone should identify themselves by their title - MD/DO (physician), PA, NP, etc. It is deceiving not to. Many states are now requiring this.
Our jobs as surgeons can bring some really tough days. In clinic today several patients told me how happy they were to see me & how much I have helped them. What they don’t know is how much they have helped me too. Today was a good day.

It never ceases to amaze me how much scar tissue can form around the femoral vessels w/ no prior open surgery, yet prior perc interventions. A good reminder to the trainees that no scar does NOT = easy cut down & exposure!

peeps: having a discussion w/ another surgeon re: tibial bypass 4 claudication in a pt w/ chronic thrombosed pop aneurysm. I can only find 1 paper from 1995 Brigham & women’s. My thought - no bypass unless rest pain. What are your thoughts?
