This is a patient of mine who is 12 years post-op from a T10-Pelvis revision PSF with a L4 PSO. Her construct looks to be doing well with overall good alignment.

55-year-old male who is one-year status post vertebral column resection for bad diskitis/osteomyelitis of the thoracic spine. A great correction of spinal deformity with great decompression of the spinal cord and removal of diseased bone through a lateral extracavitary approach.

Breast cancer can be very aggressive in 40 year old females. This patient had metastatic spinal cord compression. Doing a thorough decompression can provide some quality of life. I still reference the Patchell Lancet paper from 2005 when deciding on the best treatment.

I am sad to hear about the passing of a great mentor of mine: Dr. Ivan Ciric. He was not only a superb surgeon but also one of the greatest human beings I ever met. He taught me so much and for that I will always be indebted to him.

I wish surgeons would stop telling vulnerable patients that they must have spine surgery ASAP or they will become paralyzed. In very rare cases, that may be true. But in the vast majority of outpatient clinical scenarios, that is simply not accurate. #neurosurgery #orthopedics

Spinal fractures can be very difficulty to realign. They may have some of the same features you see with scoliosis and other spinal deformities with a rotational component, lateral listhesis, and sharp angular kyphosis.

My advice to medical students applying to neurosurgery: Find a program that provides a good amount of exposure to open vascular neurosurgery. Fewer clippings are done today and the cases that get passed on by the endovascular specialist are difficult to coil.

Spinal infections can be severe and cause complete paralysis. Doing the correct surgery to decompress and stabilize the problem is easier said than done. Not cutting corners means doing a circumferential decompression which too many surgeons shy away from.

Even though elective cases shut down during this time, I still have patients that need emergent treatment of ruptured aneurysms. I love the anatomy on these cases especially Pcomm aneurysms. #3rdnerve #anteriorchoroidal #opticnerve
