The Coroner advised that following the evidence heard during the inquest he had ongoing concerns, at a national level, regarding the communication of the restricted items regime to family and friends of patients before they visit the ward. https://t.co/LnUzw5AzbZ

The circumstances of Josh’s death also raised wider concerns in respect of restricted items on mental health wards, leading the Coroner to raise a Prevention of Future Deaths report with the Trust and the Minister for Mental Health and Patient Safety

With 40+ questions & 2 days of deliberations the jury in the inquest of Josh Sahota, who died at Wedgewood House, Bury St Edmunds, undertook their role diligently, returning a considered & concise conclusion noting a number of failings by the Trust that led to Josh’s tragic death

Good to hear through the inquest process that changes have been made at the Trust which hopefully will ensure that similar events do not happen again. West Suffolk Hospital 'sorry' after Thurston postmaster died from heart attack following hip operation

Are lives being put at risk because families are refused funding at inquests?

What a sad and shocking disparity. Goodness knows what the figure would have been, had all the Trusts responded. Huge amounts of grieving families are simply left to get on with it and face a wall of legal representation at the time they need supporting the most! Disgusting
We FOI'd every mental health trusts in England - just under half responded, we found together they spent over £4m on legal representation at inquests in a year, compared to just under £118,000 given to families in legal aid for MH inquests #legalaidforinquests @INQUEST_ORG

Nothing is more striking or distressing than when Hospital Trusts/Public bodies are represented by teams of Counsel, Solicitors and assistants and the bereaved family are simply meant to turn up and be able to engage in the inquest process against the wall of a legal team.
There is an urgent need for funding reform for bereaved families at inquests. The inequality of arms between families and the state is the most significant injustice in the coronial system. Listen to @BBCfileon4 8pm tonight #LegalAidforInquests
