This was an interesting project to do, looking back on two phases of implementing a new technology and our lessons learned in the process!.
Information and communication technology (ICT) can support the delivery of integrated and coordinated care. A case study on TAP-App describes lessons learned when using new technology to support integrated care. Check out the Infographic: @HealthTapestry

So happy to hear about @ielectHamOnt. It's time for a change to better the lives of the entire city, and current council is not doing it. My councillor has held his seat almost as long as I've been alive! (since 1988). #HamOnt

Yesterday I mentioned @McMasterFamMed's Indigenous Teaching Through Art, & now I've read about art's role in upholding colonialism while building what we now call Canada. ". a preview of the whole of settler colonialism, sordid, indelible, irreversible."

I participated in Indigenous Teaching Through Art at @woodlandcc - an intense but meaningful day where we began to more fully understand the trauma of residential school survivors (on site of a residential school), and learn about indigenous ways of knowing through art activities.
"The goal was to bring people to a place they could reverse their gaze for a moment. To be silent and to listen. The trip to the former residential school allowed a space to witness and respect each human story," said @LorrieGallant | @MacHealthSci

Very happy to be involved with @HealthTapestry. Bringing community volunteers into patient's homes creates such meaning socially. Asking "What matters to you?" instead of "What's the matter with you?" (to paraphrase @DougOliver19) brings the focus to the person. #seniors.

A nice article about @HealthTapestry in today's @TheSpec - there is so much potential for community volunteers in supporting people's health. #HamOnt #primarycare

Remembering and recognizing residential school victims and survivors with my colleagues #IndigenousLivesMatter #EveryChildMatters.
On #OrangeShirtDay we remember the children who were taken into residential schools. We want to honour their stories, celebrate their resilience, and acknowledge the lasting impact on Indigenous people across our country. #EveryChildMatters #IndigenousLivesMatter
