@seanvandiepen Correct. Ecmo is there to be used when all other conservative therapies fail. Ecmo is a tool. It is the same for all Devices. Preemptive implants are not saving lives. They cause problems and are costly

Mortality remains stalled at 50% in cardiogenic shock trials for past two decades! No improvement in outcomes with immediate ECMO in RCT ECMO-CS #AHA22. Why is our progress stuck in 1999 (SHOCK)? A few thoughts 1/

ECMO-CS. In a modest sized RCT of immediate ECMO vs early conservative therapy (with provisional MCS) in pts with SCAI D-E cardiogenic shock, no difference in death, resuscitated arest, or another MCS device.

Specific to ECMO/MCS, future trials need: 1)A priori subgroup for cardiac arrest patients or exclude arrest patients in a coma as done in ECMO-CS -In the CULPRIT-SHOCK trial, 54% of patients had CA prior to randomization. -MCS unlikely to mitigate anoxic brain injury
