2 medics I work with walking out of ER last night. Lady comes screaming into parking lot w/ 41 y/o husband in front seat in cardiac arrest. They rip him out of car, start CPR in parking lot, shock the guy once. (+)ROSC. Dude wakes up. STEMI. Hugged his kids and off to cath lab.
You’re called for pulseless unresponsive 90 year old. Family says they know there is a DNR (they are frantically looking for it). Respect their wishes. Resist the urge to intervene in the natural dying process. Do not start CPR. Sometimes doing what is right means doing nothing.

For 20 years here our daily minimum staffing has been 3 per shift.
Today, the best memo from the chief that I’ve ever had to sign off on in my career. “Effective 3/13/22 the department daily minimum staffing level shall be four (4) personnel per shift.” 👊🏼
This morning I was told of a firefighter (not from my dept thank goodness) who happens to be the junior man on his shift that complained to his union rep because his captain trains too much and he doesn’t get enough down time. A word of advice: Don’t be that guy.
I see so many cases of firefighters who stay on the job for 40 years and either die on active duty or retire and drop dead of cancer a year later. I miss the job but I have zero regrets about leaving at 29. You got the time? Get out! Collect your pension! You earned it. Enjoy it

A buddy of mine recently passed away. His viewing was yesterday. He was a past volly chief and a retired career EMT. We were instructors at the academy together. Was part owner of this sweet Seagrave fully restored antique tiller. He’d be very proud that I was sharing these pics.

I love when cops rush to a fire or EMS scene at 90 mph so they can get there and tell you to "slow it down." Hey, if you were so excited to go to a fire or a chest pain call then you should have enrolled in medic school or taken the test for the fire department.

Majority of fuel tanks are now made of high density polyethylene as opposed to pressed steel. They will soften, sag and eventually fail releasing the vehicle fuel onto the ground. This is exactly why we should be in full turnout gear and SCBA on all vehicle fires.
This is tough to watch, throw water from a far if the car fire allows, sweep the ground, use wide fog when needed, and if you can always use a back up line for safety.
Came back to the firehouse from back to back runs last night and my family was there to surprise me with a birthday cake in honor being “Welcome to the AARP” years old. Surprised we didn’t set off the smoke alarms.

Just came back from a serious motorcycle crash. Cop from another jurisdiction happened to be passing by before any local PD or EMS arrived and had two tourniquets on the guy before we got there. Without a doubt saved the dude's life. 👍👍
A per diem medic with like 6 months on the job threw a temper tantrum and resigned over the phone yesterday when the supervisor called her to reassign her to a different medic unit to accommodate someone who was doing an 18 hour shift. This is the entitlement generation.
Saw a local fire inspector today dressed in all black (tactical pants and “FIRE MARSHAL” T-shirt) with a badge on his belt. Come on bro get over yourself. Dressing like the SWAT team to go in to someone’s place of business and make sure the bulbs are all lit in their exit signs
We have paramedics who are allowed to pick up OT and work 18 hour shifts, go home, get four hours of sleep and come back in for another 12 (or 18). Pretty sure the research shows that impairment from acute sleep deprivation is equivalent to being legally intoxicated.

But the most important thing you can be is kind, compassionate, honest and empathetic.
Local ambulance services here going out of service daily for 12 or 24 hour shifts. This places an unnecessary burden on surrounding towns to cover their calls. Agencies can’t seem to find EMTs who are willing to work for sub par wages and no medical benefits or pension. Go figure
In the battle against COVID, why aren't we doing more to stress the importance of a healthy lifestyle? Nutrition, weight loss, smoking cessation, exercise, etc. The overwhelming majority of healthy people survive COVID.
Paramedics. Unless you’re going to administer meds or a fluid bolus there’s no reason to start an IV. I said what I said. Now put out your campfire, pull up your tent stakes and go to the damn hospital.
EMTs. If you’re going to request advanced life support for a patient with “respiratory distress and low pulse ox” you should probably have the patient on oxygen when the paramedics arrive 10 minutes later. Just sayin.

We take calculated risks in order to serve those we swore to protect understanding that the potential for serious injury (or worse) is ever present. Getting ambushed while fighting a dumpster fire isn’t even on my radar. Condolences to the Stockton FD and to Capt Fortuna’s family
Just had a guy who's hand was trapped in a machine. Extricated prior to EMS arrival. 100% degloved. Multiple fractures. Several amputated fingers. Go to start an IV to give this dude some fentanyl and he says, "You're not going to give me a needle are you? I hate needles!" 🤦♂️
This is an under utilized and grossly under appreciated tactic that can rapidly knock down a fire, prevents autoexposure, and buys you the time to stretch hand lines for interior ops. Especially effective in small or minimally staffed departments.
As a leader, the people who work with you deserve to know what your expectations are. How can you hold someone to a standard if they don't know what the standard is? Also as a leader, you need to know what their expectations are of you.
Back from the body shop after repairs to the hose bed. While it was there the wheels needed to be repainted too as they were also in pretty bad shape. Chief decided to go from red to black. Wasn’t sure how I’d feel but I like it.
Fire/EMS personalities on social media, "famous" instructors, podcasters and authors you've maybe met in person once or twice... They're NOT your mentors. Can you learn from them? Sure. But, you must have a personal relationship with someone in order to consider them a "Mentor."

Just informed my crew that the fire Chief is a little down today because his cat passed away. My crew then commenced firing off dead cat gifs and memes to the chief’s cell phone. There are a few things that are off limits in the fire service. Dead cats is not one of them I guess

Someone was recently tweeting about firefighters carrying extra socks and gloves in the winter. I’d say add an extra pair of underpants to the list in this department.
To whomever needs to hear it: The EMS QA/CQI process/program was never intended to be punitive in nature nor was it designed to strongarm paramedics and/or EMTs into over treating patients to generate revenue.

Pronounced a 28 y/o after ~ 25 minutes of CPR last night. Known opiate user. I’ve treated him before. After 30 years I’m jaded, salty, & plain frustrated with opiate addiction. I feel enormous dread for the families. The visible pain of his devastated parents seared me to my core
It’s perfectly OK for employees to ask for clarity about policies. “Why?” is a legitimate question. Remember that, “Because I said so” or “That’s the way we’ve always done it” are not valid responses. Managers should be prepared to objectively support all policies and directives

I would like my union to get firmly behind the concept of 100% remote/virtual firefighting. At least for the next couple months. I know the residents overwhelmingly voted in favor of "in-person" firefighting but unions need to insure the "safety" of our members

Have had a couple OD patients recently who were resuscitated with narcan but didn't return to normal baseline because they were also hypoglycemic. I think we need to start a public education campaign on the importance of eating breakfast before doing heroin
