Tipifarnib/alpelisib combination increases CD8+ T cell infiltration but upregulates PD-L1 expression in vivo. Triple combination of tipifarnib, alpelisib, and anti-PD1 significantly prolongs progression-free survival, by providing unprecedented antitumor efficacy.

These are the major outcomes:.Tipifarnib efficacy depends on CD8+ T cells in HPV-positive HRAS-mutant HNC model. In HPV-negative HNC, AKT activation and PD-L1 upregulation limit tipifarnib efficacy. Alpelisib enhances tipifarnib efficacy by blocking MAPK and AKT/mTOR signaling.

In our new publication in #neaplasia @ElsevierConnect we found that PIK3CA or HRAS mutated head and neck cancers need to be treated with anti-PD1 to prolong the response to the combination of Tipifarnib and Alpelisib.
