Step 1) ASD removal: 24Fr MitraClip Guide, 8-10mm Raptor, 20mm Gooseneck snare. I’ve done this a few times now. Use a Rat-tooth/Alligator or Raptor grasping device (prefer Raptor) and place the goose neck or home-made snare (see picture), over bioptome device.

Step 2) Redo mitral TEER on medial side A2/3 prolapse using XTW and then XT clips with 3-4+ MR reduced to 1+.

Step 3) LAA closure with 31mm WM FLX Pro via TruSteer through 20F Gore DrySeal after removal of MitraClip guide. (We do NOT want to go through this septum AGAIN!).

Step 4) Redo ASD closure with 18mm device via 9F x45 Torque through 20F Gore DrySeal. @TAVRBot

Challenging Case: Redo mTEER. 82 WM with CHF due to recurrent 3-4+ DMR after mitral TEER 7mo ago with ASD closed at index mTEER due to bidirectional shunt using 16mm ASO now p/w CHF and Afib RVR + GI bleed. CT Surgery deemed high risk due to age, frailty, anemia, double valve ds.
