If @AAOS1 wanted to make a difference for patients (and claw back relevance), it would foster reporting and censure of surgeons who deny indicated surgery on behalf of commercial carriers including medicare replacement plans for $ (similar to BS expert plaintiffs witnesses).

- Severe hip and shoulder OA are not in that category and patients should not have to fail formal therapy before being given access to a total joint for these diagnoses.

- I love therapy for conditions for orthopaedic conditions that potentially have a favorable natural history for significant numbers of patients (e.g. tennis elbow, degenerative rotator cuff tears).

- @AAOS1 can we please have clinical practice guidelines updated to indicate that therapy is NOT indicated in severe disease by HIGH-LEVEL evidence?

- Studies like this are well-done (level I!) and totally worthless. Worse than worthless, the results can be twisted by 3rd party payers to deny indicated surgery.

- Yet, we are told by ITT analysis at 24 mo that "all outcomes similar." No AT results are given at 12/24 mo. - The as-treated results for therapy at 6 months are a joke for the therapy group (ΔOHS = 4). I would like to know if any patients had change > MCID, a low-bar.

Regarding @NEJM "Total Hip Replacement or Resistance Training for Severe Hip Osteoarthritis" ( https://t.co/etudNkhKqn): a rant. - Can an ITT analysis for this study with 21% crossover to THA by 6 mo have any validity? How about 59% and 77% crossover to THA at 12 and 24 mo?
