I’ve been keeping this one to myself, but last year I was exposed to HIV from a needle stick injury. It made me realize that, as a physician not dealing with HIV regularly, I really didn’t know much about it at all.

If you wanted to see the entire eclipse over North America, you’d have to fly from Mazatlan, Mexico to Bonavista, Canada averaging 2,048 mph (3297 km/hr). You’d have to start at about 1,500 mph and end at over 5,300 mph, experiencing totality for 1 hour and 39 minutes.

At this point anything goes. Either physicians evolve & over see the tech or get pushed out. My guess? Physicians in the post AI world will still be employed, but largely as scapegoats for liability. Because at the end of the day, some high earner has to be around to get sued.

When faced with the option of simply no image interpretation (hospital grinds to a halt) vs liability of trusting new AI tech, hospitals will choose the lesser of two evils. This will be the foot in the door moment for AI radiology companies & will be Pandora’s box for radiology.

We’re already seeing this without AI; radiology volumes are currently at an all time high and some hospitals literally cannot staff radiologists. This problem will worsen exponentially. At about this time, AI image interpretation will be starting to incorporate into PACS systems.

As we’re already seeing, midlevels with far less clinical experience, in combo w/ conservative computer-aided recommendations, will result in worsening of the already bad over-imaging epidemic, increasing demand for radiologists to interpret the onslaught of inappropriate studies.

Nurse practitioners/PAs, equipped with LLMs as part of the EMR, will be hired in larger numbers because hospitals won’t be able to help themselves when tempted by the prospect of massive cost savings by reducing the number of physicians on payroll.

A radiologist’s predictions regarding AI and healthcare in the USA:. Large language models will be incorporated into electronic medical records (EMRs) well before radiology systems (PACS), jeopardizing internal medicine doctors’ jobs far before radiologists’ jobs.
