Today's report makes a good case for an effective oversight mechanism for domestic homicide reviews/domestic abuse related death reviews.
When someone loses their life to domestic abuse, it is an appalling failure of our society, the state and the systems designed to keep us safe. With a robust monitoring system for domestic homicide reviews we can prevent future deaths and tackle abuse.
