図はARへの推奨ですね。 J-ValveとJenaValve TrilogyがTAVI for pure ARに対するCEマークを取得していますが、FDA承認はまだです。後者はARTIST trial (NCT06608823)の結果次第で今年中にFDA承認…と期待されています。 Severe ARもUnder treatmentが問題なので、COR IIbはそこも反映したのでしょう。
・二尖弁へのTAVIがIIb へ。 これはなかなか攻めた推奨。今のところ日本はおろか広い一般使用として世界中でも承認されたデバイスはないと思うのですが（あったら教えて）、Off-labelでのAS用TAVI弁の使用にも言及している。 ペースメーカーが多いので注意しろとも

・TAVI弁の耐久性 ・劣化したTAV弁がclinical outcomeに与える影響 ・劣化の予測因子 をreal-world registryのデータを用いて解析した論文です。 Discussionで触れましたが、現行のTAVI populationでどこまでecho F/Uを完遂出来ているのか、するべきなのか、を考えさせられる研究でした。

EARLY TAVRの真に凄いところは、無症状とはいえsevere ASにがっつりtreadmill をかけて、偽無症状患者じゃないことを堅実に確認したこと（9割以上）。他のほとんどのearly interventionのtrialが避けている中、これを徹底したのは本当に凄い。
