Manufacturers - this is end user info you are getting here. Grabbing what you think is saline. Human factors nightmare. Can you all get together & universally decide how the packaging should look? I am genuinely interested in how many drug errors occur due to similar packets

I think it was Audit Commission. I tried to find a copy a few years ago and couldnt. It was pre www so if we can get anything it may be scanned hardcopy. From the AC itself or from the British Library or even National Archives???
