We absolutely need to get better at ensuring advanced care planning is done when patients are able to make informed choices and be fully engaged in this discussion. There are too many occasions when it is just too late. This is not acceptable 😞
People with an Advanced Care Plan have better outcomes including more likely to die in preferred place, better symptom control and better overall quality of care #GSFConf19
