Urine microscopy: A shamrock shaped oval fat body. Identified by R1 two weeks into her IM residency! (McGill grad, just saying). Pt with nephrotic syndrome, anasarca, serum albumin 16 mmol/L and urine protein 12 g/g and minimal change on renal biopsy.

Feb 23, 2023 was the 75th anniversary of the first dialysis in Quebec. In Oct 1947 the Kolff dialysis machine arrived in Montreal, followed by Dr. Nanny De Leeuw, a student of Dr. Kolff. She and her team performed the 1st dialysis at the Royal Victoria Hospital.

Exactly 1 year ago on the today anniversary of our potassium lectures! Hyperkalemia ECG pre and post glucose and insulin shifting. The patient a year later is well, with good low K dietary adherence.
Thought if you all tonight seeing a dialysis patient in ER with K= 7.9 non hemolyzed. ECGs are pre and post glucose and insulin. Came in not feeling well… now getting dialysis
