Trimethoprim inhibits amiloride-sensitive Na channels in the distal tubules affecting the electrochemical gradient for potassium excretion. If TMP-SMX is the DOC but treatment refractory hyperkalemia arises, fludrocortisone may be utilized to manage the ⬆️K and continue TMP-SMX.
