The CQC did not comment on the repeated warnings and allegations it received - but told us that Ms Slaney-Summers’ actions were “disturbing” and it was “considering what further regulatory action we may take” to protect care home residents

After watching coverage of Rita Barnsley’s case on @Channel4News in October, another ex-Amberley worker - who complained to the CQC but felt driven out of her job by Slaney-Summers - said: “I just cried to my husband. Maybe I could have held on for longer and done something"

Julia Botfield - chef at Amberley for a decade - described Slaney-Summers as a “narcissistic bully”, claiming that she would often witness her shouting at residents with dementia. She went to the CQC in early 2020 after an alleged incident which left a man "screaming in pain"

Eaglesfield said Slaney-Summers messaged after her mother’s death to ask: “Did you inherit the house?” She criticised the CQC for failing to “dig a little deeper” into Slaney-Summers - arguing they could have uncovered evidence before she was able to prey on Rita Barnsley

Sarah Eaglesfield said she complained to the CQC in January 2019 while her mother was in Slaney-Summers’ care in Cedarwood Care Centre in Birmingham. She said Slaney-Summers would attempt to “drive a wedge between me and my mother when there was a huge inheritance at stake”

The CQC received at least EIGHT separate complaints about Slaney-Summers between 2014 and 2021 - spanning FOUR different care homes. These related to allegations of financial abuse, physical abuse and unsafe practices during COVID

On the day Slaney-Summers was jailed for five-and-a-half years, we can reveal new allegations including: 🚩 physical abuse of an elderly man inside a home 🚩 make-up being used to cover up a resident’s injuries 🚩 positioning herself as next-of-kin for another wealthy client

A jury found: 🔴 failure to conduct further tests on Mia “possibly contributed” to her death 🔴 “inappropriate level of assurance” that full physical tests had been completed 🔴 lack of robust communication and risk management inside the unit where Mia fatally harmed herself

That critical information changed the course of the 10-day jury inquest into Mia’s death. A pathologist revealed the results in the second week of proceedings. Mia’s death was now ruled to have been caused by “acute psychosis”, triggered by “autoimmune encephalitis”

What was not known at the time was that Mia was suffering from autoimmune encephalitis - a treatable physical disorder resulting in brain inflammation that can cause extreme psychiatric symptoms. Queen’s Medical Centre had failed to conduct further testing and diagnose Mia

On 29 January 2024, Mia was able to inflict a fatal injury upon herself inside her room at the Becton Centre. In a matter of weeks, Mia had gone from a healthy schoolgirl to suffering extreme psychosis and then dead inside an NHS children’s hospital

Mia was not kept safe at the Becton Centre - she was able to seriously self-harm on at least four occasions. Her mum Chloe arrived one day to find her daughter with two black eyes, a swollen forehead and a bald patch after she pulled her own hair out
