During his inquest they found that staff had falsified observation documents. For several says after he died he was reported as being 'well' and 'eating his breakfast'.

The family of Winbourne Charles found that staff failed to do observations on him when he was known to be suicidal. He took his own life in 2021.

Her family later found out she’d tried to harm herself using plastic bags or bin liners on 18 previous occasions during her admission. The bin liner in the bin she had access to wasn’t removed, despite the risks it posed.
