Errors happen in every industry, but in aviation, the stakes are high. Avoiding pilot error doesn’t fix the problem, it leaves the old narrative unchallenged. Every error is an opportunity to ask: 🔹 Why did this error make sense at the time? 🔹 What conditions allowed it? 🔹

How to talk about pilot error the right way: ❌ “The accident was caused by pilot error.” ✅ “The crew’s decision-making, shaped by workload and procedural gaps, contributed to the accident.” This keeps the focus on learning and system improvement, rather than blame.
