2023 MIPS and PI have measures for connecting to QHINs (that was news to me), but CMS is asking how to get payers involved, and how patient access could be simplified - I look forward to the comments on those two.

Payer to payer and new provider to payer exchange uses bulk FHIR. Good to see some consolidation around methods here.

Patient auth rule is replaced with the same thing + a few extra data points like prior auth. Goalposts have been moved to Jan 1 2026. Not sure how this changes things for the companies in that space - could mean that being the Plaid for payers means more screen scraping.

DaVinci prior auth specs are proposed to be mandated, starting January 1, 2026. But you still have to use X12 278. Can't have everything. MIPS and Promoting interoperability are being used as incentives, so likely to see effort from providers in addition to payers.
