Proximal tibial fracture in adolescent can be associated with popliteal artery injury. Neurovascular exam of the affected extremity is of utmost importance. If popliteal artery injury is present, urgent ExFix, fasciotomy and vascular repair should be done.

Pediatric traumatic bone defect can be treated by bone grafting or bone transport. Rarely, I will use "allograft". It carries higher incidence of infection, but can avoid lengthy recovery and ExFix inconvenience. Good healing potential in kids allows the usage of this technique

@BucksHandExpert @northwoods1980 What do you think of this? 40M; initial open fracture; part of polytrauma; treated by a colleague with wires and an exfix; exfix came off at 6 weeks. This XR is at 12 weeks.

After evaluation in the ER, the patient is taken to the OR for emergent external fixator, irrigation and debridement, and wound closure. A CT scan is performed after I&D and ExFix stabilization. Here are the CT 3D reconstructions.

Prayers go out to Texans WR Tank Dell after that injury. On first look this appears to be a possible compound distal femur fracture. There will be emergent surgical intervention. This could require an ExFix at first then ORIF vs Intramedullary fixation for definitive management.
